Forums theme: Add login link/form to header

There is currently no reference to a login form. We should at least add a link to ​https://wordpress.org/support/bb-login.php and/or implement a login form like we have on ​https://wordpress.org/support/.

Seems like the logout link doesn't work.

bbPress.org/BuddyPress.org use the bbPress shortcode [bbp-login] on a page ​https://bbpress.org/login

We also move the admin toolbar from the top of the page to a navbar menu, this removes some WordPress Admin menu links and also adds links to the login/logout user profile links etc:
sites/trunk/buddypress.org/public_html/wp-content/plugins/bbpress-org/toolbar.php

Maybe go down this path instead?
