Add 2013 WP survey to the header on dotorg

Description
I wasn't here last year so I'm not sure exactly how this is done, but we need to add the 2013 WordPress survey to the dotorg header (as a banner? how was this done last year?).

We'll also do a push on social media and on the WP news blog – or a different blog? it wasn't on the news blog last year. I'll need access to those accounts (Twitter, Facebook). And we'll be doing an email blast, but I have to write the email still.

Now — assuming the survey is "final", and unless it is to be timed with an announcement. This is done yearly and the data collected is used in part for Matt's WCSF keynote.

BTW, the header has changed a LOT since last year. There is not the same amount of space and such in it, so I can't add it in quite the same way. Suggestions on where to fit it in there would be appreciated.
