Screen reader text for jump to links

Description

The jump to links creatd by the table of contents only have the "#" character as text. This character is spoken as "number". We should provide screen reader text to make it more accessible.

The 1861.patch provides screen reader text that's hidden with the class .screen-reader-text. This class is also used by the callout boxes. I assume this class is globally available for use.
