Screen reader text for jump to links

Description

The jump to links creatd by the table of contents only have the "#" character as text. This character is spoken as "number". We should provide screen reader text to make it more accessible.

The 1861.patch​ provides screen reader text that's hidden with the class .screen-reader-text. This class is also used by the callout boxes. I assume this class is globally available for use.
