No-Follow links in readmes

The current plugin pages nofollow all the links.

The author and donation links on the new plugin pages are nofollowed, the in-content links are _not_.

As much as we hate this, we need to nofollow all the links in readmes, as some rather scummy 'SEO consultants' will leverage that to bolster their search results in the googles and other places.
