I've seen this a few times and others have reported it both on the make P2s and on the wordpress.org/news blog, which makes me think it's a system- or WordPress-level issue.

Sometimes when you upload an image to a dotorg blog, it fails. By that, I mean the image is present in its full form but the various sizes of it don't get created. WordPress seems to think the thumbnail exists in these cases and tries to place it, resulting in a broken image.

I've seen this for a few weeks, but can't reliably reproduce it. Maybe one in five images have this problem. Sometimes more frequently, sometimes less.

I can reproduce this consistently when uploading galleries. The make blogs, especially make/flow, really need to be able to upload images reliably.

This was replication lag. Our copy of HyperDB wasn't properly sending reads to masters after a write. Fixing this should fix a host of other "gremlin" issues we see on WordPress.org.
