To report an Accessibility issue you’ve encountered in WordPress or on WordPress.org, please see the Accessibility Handbook on <a href="%s">Reporting Accessibility Issues</a>.
I think this is not correct, bacause the link brings you to https://make.wordpress.org/accessibility/handbook/reporting-issues
and there is no handbook but it's a sub part of the handbook. I propose to just use:
To report an Accessibility issue you’ve encountered in WordPress or on WordPress.org, please see <a href="%s">Reporting Accessibility Issues</a>.

"on" means "on the topic of" here, not that the page itself is the handbook.
uh, oops, yes, now I see. But if I found it misleading, some others could be trapped the same way, so let's improve it, like you said.

A note:
The heading is "Report accessibility issues" while we link to "Reporting Accessibility Issues"
The existing string looks good to me, it explains where the link goes. "on" means "on the topic of" here, not that the page itself is the handbook.
It could probably be made a bit clearer with:
"please see the Accessibility Handbook page on <a href="%s">Reporting Accessibility Issues</a>"
.or:
"please see the <a href="%s">Reporting Accessibility Issues</a> page in Accessibility Handbook"
.
