URLs like ​https://apps.wordpress.com/img_2389/ shouldn't be exposed to / in search engines.

Arguably, they're not useful to users, either.

At best, redirect requests to these templates (via a 301 header) to the URL of the media asset in question.

At least, include a meta robots directive on these templates with a noindex, follow value.

Also worth noting that this is the default WordPress behaviour, attachment pages are a thing.. Perhaps this is worthy of a core ticket discussion?
