Improve the page title and meta descriptions of theme author archives

Theme author archives (such as ​https://wordpress.org/themes/author/dans7919/) currently have a title structure of {{AUTHOR}} | WordPress.org.

This should be changed to Themes by {{AUTHOR}} | WordPress.org.

The meta description should be changed to "See all WordPress themes developed by {{AUTHOR}}".

We should always use the 'clean' version of the author name, rather than the slug. E.g., for ​https://wordpress.org/themes/author/d5creation/, that's D5 Creation, rather than d5creation.
