Update forum disabled page to include Forum Password Resets email

Description

Can the "Your Account Has Been Disabled" page be updated to inform the user to mail "forum-password-resets@…" for any communication regarding their banned account?

When a forum user's actions cause them to have their account blocked and they log onto the forums after, they are directed to a "Account Disabled" page. This sometimes causes them to create a new account just to post to the forums a topic like "Why was my account disabled?"

Which promptly gets their new account banned as well as bypassing a ban isn't appropriate. Users are banned for cause.

Blocked forum accounts cannot create a Slack account so the only way they can appeal their ban is to email the forum-password-resets team. That does happen but it's very sporadic as I believe the page does not inform the user today. Sometimes they email the plugins team incorrectly.

If the page has that added to it then more blocked users will be able to appeal their forum status to the correct people.
