User Profiles: Audit Log user role changes

When a user's role is changed via editing their profile, we need to log the action (who did it and when).

This would be for all state changes (change someone to a mod, block a user, etc)

Also we need #4192 so that if you change the state, it's required to leave a reason why.

This was done in [10595] for blocking a user.

We don't have anything for when a user is promoted to a mod, but we're trialing other audit logging plugins for that.

I'm going to close this ticket as fixed for now based on the above commit.
