Re-order form fields

Description

When I want to upload an image, I ALWAYS start with the upload, then go to the top of the form and do Description, and then skip down to the checkboxes. I want to see the image first, and the describe it. I recommend we move the upload box above the description box for workflow.

In 11690:

Photo Directory, Uploads: Move file upload input field to top of submit form.

Better to front-load required fields (which file upload is), whereas description (which previously appeared before it) was optional. And it makes better sense to progress from picking/seeing the photo and then describing it.
