The styles for this appear to be 100% the Jetpack Forms Contact Forms Grunion CSS, and not WordPress.org / 5ftf.

I'm unable to locate an upstream Jetpack issue, nor can I see what's immediately wrong with the CSS.

Additionally; in future, please consider uploading the image to the Trac ticket after ticket creation, as including links to image hosting platforms and google drive can result in broken links or no inline context in the future.
