On the WordCamp placeholder page the contact form was overwritten by the form we added to one of the internal pages. ​http://oc.wordcamp.org/2015.

Yeah, I was afraid that would happen :|

​#102-jetpack isn't fixed yet, so I think for now we'll just have to work around it. You can do that by creating a temporary page with a contact form that you want to use, and setting the other form(s) to drafts. Or, you could make sure that the page you want to use has a lower ID than the other pages.
