Make the request a theme button able to appear in a page

We plan on making a two step process by having a page that then will link to the request a theme button as seen in the widget used now on make.wordpress.org/themes.

As it's a widget, we want to know if this is possible? If not, can we add an additional page to the process that makes people check before clicking. We'd want to add content to that page.

I copied the code from #762 and add a shortcode. I was not able to full test the code as it had some constants and references to code on WPORG which I don't have access to.

I created a gist of the initial version of site-themes.php and then added my changes so that you can see the changes that I made.
