Log who made changes to Polyglots caps

Description
It has now happened on a number of occasions where there were changes to Editor caps (PTE / GTE) that cannot be traced to any individual who made changes.
This issue could be resolved by including the username of the person who made the change in the logs, to enable the Polyglots Global team to follow up.
While an optional reason would be nice, just having the username is more urgent and should not be very complex to implement.

As this should not require changes to the UI, while an optional reason would need an additional field in /wp-admin/.
