Make sites should let you link to all tracs

Description

Currently, if you type in #123 on a make site, it assumes you want to link to the core ticket. Much like intertrac does for our trac instances, we should let you type #123-meta (for example) and link to the relevant meta ticket.

(Note, shortly after posting this ticket, make/community and make/meta will be changed to link to meta tickets by default. But they should still be able to go to #123-core and link to the relevant core ticket.)

There's code for this on WordPress.com, we just need to copy it over and adapt it.

Note, shortly after posting this ticket, make/community and make/meta will be changed to link to meta tickets by default.

The #123-meta example is based on the Slack implementation, ie #123-meta does not link to #123 here on trac

Intertrac uses the trac name to prefix the ticket number, #meta123, so #meta123 links to #123, #wp123 links to core ticket 123, #bbpress123 etc

The Intertrac format has been around forever, not sure why Slack was implemented differently :/

It would be great if we had a standardised format that can be used across any w.org resource :)

Both.

It only allows 4-5 digit numbers, though, so #123-whatever won't work.
