Make sites should let you link to all tracs

Description
Currently, if you type in #123 on a make site, it assumes you want to link to the core ticket. Much like intertrac does for our trac instances, we should let you type #123-meta (for example) and link to the relevant meta ticket.
(Note, shortly after posting this ticket, make/community and make/meta will be changed to link to meta tickets by default. But they should still be able to go to #123-core and link to the relevant core ticket.)

The #123-meta
example is based on the Slack implementation, ie #123-meta does not link to #123 here on trac
Intertrac uses the trac name to prefix the ticket number, #meta123
, so #meta123 links to #123, #wp123 links to core ticket 123, #bbpress123 etc
The Intertrac format has been around forever, not sure why Slack was implemented differently :/
It would be great if we had a standardised format that can be used across any w.org resource :)

Fixed in [dotorg12885].
There's code for this on WordPress.com, we just need to copy it over and adapt it.
