Theme report page is not working

Description
The theme report page https://make.wordpress.org/themes/report-theme/ is not working. The submit button is not working and shows the console error.
An invalid form control with name='rep-theme' is not focusable.

The https://make.wordpress.org/themes/report-theme/ page was not designed to be accessed directly. It's supposed to only be accessed via the report button on individual themes.
I've re-enabled the display of the hidden Theme URL field, so that it can be filled in when accessed directly.
