Plugin Directory: Release Confirmation Email lacking some important info

Here's the current email:

Howdy [NAME]

Release confirmations are now enabled for [PLUGIN NAME].

This means that each time you release a new version of [PLUGIN NAME] you'll be required to confirm the release by following a link in an automated email.

Issues are:

  1. Doesn't link to the plugin. This needs to happen. People have waaaaaay too many similarly named plugins out there
  1. Doesn't say WHO turned it on. So you don't know who to poke at about this.
  1. Links to the handbook which has no info on this because we've yet to write any. I quickly stubbed out https://developer.wordpress.org/plugins/wordpress-org/release-confirmation-emails/ just now, but we need to write up the requirements (which I think are 'you have to use tagged releases')
