Add ability to filter feedback on the form

Currently, when there are different forms for several related areas it is hard to find what you are looking for.

The ability to filter based on which form the feedback was left in should make it easier to find what you are looking for.

Which forms is this about? If it's the [contact-form] shortcode, that's part of Jetpack, so you'd need to suggest it to them.

If it's some of WordCamp.org's custom code, then ​feature requests need to be proposed on the Community Team's p2 first, and approved by them, before the Meta team does anything with them.

Closing for now, but this can be re-opened if the Community Team approves it.
