Error while uploading the photo

Description
So whenever I’m trying to upload a new photo in the photo directory, my process got completed, but the file actually didn’t even upload their after the submission. My screen appears white and the original file is not selected after I refresh the site. It asked me to submit it again and again the same problem Encounters me.
