Plugin Review Submissions - Allow authors to add comments about their new plugin version uploads

When a plugin author uploads a new plugin version on the submission page, the plugin author can't add a note/comment/description to give more context to their newly uploaded plugin.

While we would still expect them to reply to the email confirming the changes, we discussed that internally as part of the Plugin Review team, and it would be good to have something that could allow additional context in case there are issues with the email replies.

Early implementation added above, I'm leaving this open for feedback from reviewers once they get to experience it.

It's likely that we'll need to clarify what these fields should be used for.

Plugin Directory: Fix an issue where some ZIP uploads would not properly notify reviewers in Helpscout.

Helpscout rejects API requests which specify a firstName or lastName that is 0 or 41+ characters long.
